To preliminarily determine if you are eligible for the VR study, we first need to ask a few brief questions. None of the responses will be recorded and you will not be asked to provide any personal information. If your answers indicate that you may be eligible for the study, then we will provide a short form that your doctor must complete to confirm eligibility.
